1. Return to School Survey
Dear Families,
Please spend a moment to share some quick feedback on how your child(ren) is (are) feeling about their return to school. https://forms.gle/Ct2xW4Cm8pbDWzMn6
2. School Start Up Forms
Please visit our website to review our school start up package and key school information please visit our website at http://www.yrdsb.ca/schools/cornellvillage.ps/Pages/default.aspx
Parents/guardians received an email from YRDSB Update with important school start-up forms to complete. It is important that these forms are completed as soon as possible at the start of the school year as they contain important information and permissions.
The forms can be found at https://startupforms.yrdsb.ca/.
A link to the forms can also be found on the Family Resources page of the Board website at www.yrdsb.ca/familyresources. Click on the Need Help link beside Family Startup Forms for additional information on how to log in to the Student GAPPS account to complete the forms.
Please note:
You must be signed in to your child’s student GAPPS account to complete the form. Please remember to sign out of one account before signing into another. Additional information is available on the Board website on how to login to the student account to complete the forms.
The forms should be completed for each student, whether they are attending school in person or participating in remote, online learning.
If you prefer to complete the forms on paper or have questions about the information in the forms, please contact the school office.
Parents/guardians must complete a separate form for each child attending school. Please ensure you log out of one student account before logging into another. You will not be able to submit the form twice logged into the same student account.
3. Short-Term Student Absences
Please be advised if a student is at home because of they are experiencing symptoms, whether or not these are COVID related (i.e. they did not pass the screening tool and you are awaiting covid-testing results) or not, this will be considered a short-term absence. Some students may also be staying at home while their symptomatic siblings are awaiting COVID test results.
If the class is a Hybrid classroom, students MAY join their classes remotely. If the classroom is engaged in the face-to-face learning model then please reach out to the classroom teacher who will be able to provide the student with work to complete through the google classroom. Please understand that learning activities will likely not be posted right away for last minute absences as it will take face to face staff time to gather and post resources. Typically, learning for Literacy and Math will be posted by the end of your first day of absence. Some junior or intermediate classrooms may have some additional subjects added by prep teachers if google classroom is a regular platform for learning.
You will find the class list below indicating the teachers name, their email address along with the learning model the students currently engage in.
4. Health and Safety:
The Ministry of Education and York Region Public Health have indicated that the requirement for elementary and secondary students to complete the daily on-site confirmation of self -assessment screening will not be extended beyond the initial two-week window. As such, beginning Monday, September 27, families/students are not required to complete the confirmation form each morning.
Please note: Daily self-assessment screening for Staff and Students continues to be required.
Parents/Guardians of Elementary students are required to complete on-line self-assessment COVID-19 School and Child-Care Screening Tool (please click on Check for Symptoms Daily), prior to their child entering the school (*this is the York Region Public Health Screening guidelines, not the Ontario Provincial screening tool). You can also print the attached Screening Tool to use daily. Note that these tools do get updated so using the online tool will be the most up to date. The latest version is now SEPTEMBER 27th
Any students who are experiencing symptoms consistent with COVID-19 as identified in the screening tool (not related to a COVID-19 vaccine) or answer yes to any of the screening questions, must not enter any school and follow the guidance in the screening tool.

Message to Families from YRDSB
Subject: Asynchronous Learning Day / Schools Closed to Students - Friday, October 15, 2021
Dear families,
We want to thank families for their patience and support as we have started the new school year and implemented the hybrid learning model. Over the past month, our Board of Trustees and senior staff have had an opportunity to hear feedback from families and educators and observe classrooms.
On Friday, October 15, school staff will engage in professional learning on the hybrid model.
Schools will be closed to students on Friday, October 15 . Please do not send your child to school. Asynchronous (not live) learning activities will be provided to students in order to support their continued learning.
This additional professional learning will enable us to share effective practices and support, and be responsive to some of the feedback we have heard. We are committed to creating learning environments that enable all of our students, both in-person and remote, to be successful, and feel connected and supported.
We want to acknowledge the professionalism and dedication of our educators as they implement this new model, which enables students to remain connected with staff and peers in their home school, and allows for greater flexibility between remote and in-person learning options, and maintaining the continuity of learning.
We apologize for any inconvenience this may cause for families and thank you for your patience and support.

11. Canadian Medic Alert Foundation
Safeguarding your child’s health is our priority. With the rise in childhood diabetes, asthma and allergies, it’s estimated that 1 in 5 children has a medical condition, allergy or special need that should be communicated to emergency medical personnel in an emergency.
For this reason, Canadian MedicAlert Foundation created the No Child Without program. No Child Without provides students, from the age of 4 until their 14h birthday, with the 24-hour protection of Medic Alert emergency information services, at no cost.
During the day your child interacts with many individuals who may not know their health condition. It’s important to endure that your child’s vital medical information is readily available and can be accurately communicated in an emergency. Having MedicAlert identification can alert school staff, friends, coaches and others about your child’s medical condition or special needs when it matters most.
